Your header should be typed in like this example: Joe Blow If you don't fill in the From: header field, the last remailer will use its default. If the last remailer in the chain doesn't support From: headers, it will use its default. Since there are only a few remailers supporting user From: headers, you might be stuck with using a non-reliable remailer. If the latency or uptime for one of those remailers isn't up to your standards, I'd just leave the From: field blank. If using the From: field is important to you though, select a good remailer based on the stats. A message that gets through with no user From: header is better than a message not getting through because a poor remailer was chosen.
