It is "Superkaramba" which has a huge variety of different monitoring and other types of themes such as clocks available. There are others in the set I have shown which is called "Easy Monitor" theme.

Not all of the themes will work flawlessly so it's best to choose from those that have a reasonably high 'score'.

Also, as with all things Linux, you can simply edit the theme file to change the colors, background image (or remove it altogether) or the content and/or layout of what is displayed.

You'll need to turn on the compositing feature (for Q4OS) via the system settings (Settings - Desktop - Window Behavior - Translucency) in order to avoid a black box appearing behind the theme.

Initially, you install the base program (superkaramba-trinity) using Synaptic Package Manager.
